These Terms & Conditions ("Terms") govern your access to and use of the website https://restore-reform-pt.com (the "Site") and the services provided by Restore & Reform, LLC (doing business as Restore & Reform) ("Restore & Reform," "we," "our," or "us"). By accessing this Site or scheduling services with us, you agree to these Terms.

If you do not agree with these Terms, please do not use the Site or schedule services.

Scope of Services

Restore & Reform is a cash-based healthcare and movement practice located in North Carolina. We provide physical therapy, Pilates, and related movement-based services focused on education, active participation, and long-term outcomes.

Information provided on this Site is for general educational purposes only and is not a substitute for individualized medical advice, diagnosis, or treatment.

Eligibility

By using this Site or scheduling services, you represent that you:

  • Are at least 18 years of age, or

  • Are a parent or legal guardian consenting on behalf of a minor

Scheduling & Appointments

Appointments may be scheduled online, by phone, or via email. Appointment times are reserved specifically for you.

Because we operate as a one-on-one, cash-based practice, missed appointments, late cancellations, and late arrivals significantly impact our ability to serve other clients.

Cancellation & No-Show Policy

We require at least 24 hours’ notice to cancel or reschedule an appointment.

Cancellations made with less than 24 hours’ notice, or failure to attend a scheduled appointment, will result in:

  • A charge of 50% of the single-session fee, or

  • For package clients: forfeiture of one session from the package

Late arrivals may result in a shortened session in order to respect scheduled appointment times for other clients. Full session fees still apply.

This policy applies regardless of reason, including illness, travel delays, or scheduling conflicts, unless otherwise determined at our discretion.

Fees & Payment

  • All services are self-pay and payment is due at the time of service unless otherwise arranged in advance.

  • We do not bill insurance companies directly.

  • Receipts or documentation may be provided upon request for personal records or potential self-submission to insurance.

Prices, packages, and service offerings are subject to change with notice.

Packages & Expiration

  • Session packages are non-transferable and may not be shared between individuals.

  • Packages must be used within the timeframe specified at purchase.

  • Unused sessions are not refundable once services have begun, unless otherwise stated in writing.

Communication

By submitting forms through the Site or scheduling services, you consent to receive communications related to your care, scheduling, or practice updates via email, phone, or secure patient portal.

You may opt out of non-essential communications at any time.

Use of Website

You agree not to:

  • Use the Site for unlawful purposes

  • Attempt to gain unauthorized access to the Site or its systems

  • Copy, distribute, or reproduce Site content without written permission

All content on this Site is the property of Restore & Reform, LLC unless otherwise noted.

Participation in physical therapy, Pilates, and movement-based activities involves inherent risks, including but not limited to soreness, strain, or injury. By scheduling services, you acknowledge and accept these risks and agree to follow guidance provided by your provider.

You are responsible for disclosing relevant medical history, conditions, or changes in health status prior to and during services. Medical clearance may be required for certain conditions, post-operative care, or higher-risk activities.

Specific risks, benefits, and alternatives will be discussed as part of your informed consent during services.

Limitation of Liability

To the fullest extent permitted by law, Restore & Reform shall not be liable for any indirect, incidental, or consequential damages arising from use of the Site or participation in services.

Nothing in these Terms limits liability where prohibited by law.

Privacy

Your use of the Site and services is also governed by our Privacy Policy, which describes how information is collected, used, and protected.

Changes to These Terms

We reserve the right to update these Terms at any time. Changes will be posted on this page with an updated effective date. Continued use of the Site or services constitutes acceptance of the revised Terms.

Force Majeure

Restore & Reform shall not be held responsible for cancellations, interruptions, or inability to provide services due to events beyond our reasonable control, including but not limited to severe weather, natural disasters, power outages, public health emergencies, or other unforeseen circumstances.
